August and Helen's Marriage

Here are the documents for the marriage of August Kozub and Helen Pollack:




On the marriage license, it says August's parents (Alexander and Irene) are living in Wilpen, PA and Alexander is a reverend. The marriage license was signed Feb. 18, 1919. Also, Helen's mother's name is Petrosky. But here, it says "Peterson." It must have been "Americanized." Interesting. That is helpful to know as I get started with her side of the family. Also, I learned what town in Ohio Helen was born in. . . Youngstown (which is where August later dies). August and Helen were married Feb. 18, 1919 in Duquane?, Pennsylvania.

Irene Kozub's Death Certificate

Yeah! I finally got it! The following was issued to me on August 5, 2013 by the New Jersey Department of Health, Office of Vital Statistics and Registry:








It says:
Date of Death: Dec. 11, 1942.
Place of Death: Middlesex County, Metuchen Township?, at a nursing home.
Former or Usual Residence: 173 Broad Street, Perth Amboy, Middlesex, New Jersey
The informant was her son Anthony Kozub.
It says she is a widow of Alex. That she was 66 when she died. That her birthplace is C. Slovakia.
That her father is "Ges?" Kelley, born in C. Slovakia. That her mother's maiden name is "unknown" and her birthplace is C. Slovakia.
It says she is buried in the St. John Cemetary.

No new information, except that her actual place of death was in a nursing home. I find it interesting that Anthony did not know his grandmother's maiden name.
